CWHC’s clinic hours are changing
Beginning November 1, 2021, our primary care, gynecology, and trans health services will be available on Mondays through Thursdays from 9-5pm. Appointments, prescription refills, and follow up will not be available on Fridays. Refill requests typically take up to 3 business days to be completed, so clients are encouraged to reach out early each week about refills requests, or questions. Please note that Friday hours remain the same for our other programs - Counseling, Integrative Health, etc.
About Primary Care
CWHC’s provider team includes Nurse Practitioners who are offering primary care to women and trans-identified adults. Primary Care services are available to individuals who are age 18 or older.
Primary Care includes general health concerns, such as (but not limited to):
Allergies
Anxiety and/or Depression
Asthma
Diabetes
Digestive Problems
Headaches
High Blood Pressure
Joint Pain
Physicals
Respiratory Infection
Screenings
Sexually Transmitted Infections
Stress-Related Illness
Thyroid Conditions
We are not an immediate care facility. If you have a medical emergency, please dial 911 or go to the nearest emergency room.

Same day visits
Same day appointments are limited and subject to availability. These are by appointment only, so please call to schedule rather than walking in.
Same-day appointments are only for returning clients. Because they are focused appointments, they are not long enough for new clients to establish care.
Some types of concerns appropriate for same-day visits include:
Allergies (seasonal)
Breast lump check
Minor burns, including sunburn, and other skin concerns, such as rash, hives, or irritation
Coughs and flu-like symptoms
Urinary tract infections (UTI)

Who should see a Primary Care provider?
Everyone! Your primary health care provider is the provider that establishes long-term care with you as your health needs change and evolve. Come see us for preventive care to keep you well or for acute or chronic health concerns. Addressing any general health concern begins with a visit to your primary care provider. The Nurse Practitioner will work with you to address your concern or determine if a higher level of care is needed. If so, we will help to link you to the appropriate specialist.
What is a Nurse Practitioner?
A Nurse Practitioner is a health care provider who manages acute and chronic health conditions. As an Advanced Practice Nurse, a Nurse Practitioner can diagnose and treat clients, as well as provide prevention-focused health education and screenings. Depending on the specialty of the Nurse Practitioner, they can provide comprehensive primary care and/or care for specific populations or specialized clinical areas such as Women’s Health. Our Nurse Practitioners are experienced in primary care across the lifespan.
