New and returning clients can schedule an appointment for any service by calling CWHC at (773) 935-6126 during clinic hours (Monday through Thursday, 9am - 4pm).

Returning Gynecology and Trans Health Clients Only

Returning Gynecology and Trans Health clients can schedule an appointment online through the Athena Patient Portal. For instructions on setting up your portal and logging in, visit our Client Resources page. For instructions on how to use the online scheduler, read our scheduling guide.

Online scheduling through Athena is available at this time for returning clients. New clients must call (773) 935-6126 to schedule an appointment. 

Returning clients are welcome to schedule appointments either by phone or through Athena. CWHC has introduced online scheduling as another option for accessing care, not to replace scheduling by phone. 

Returning clients can use online scheduling for Gynecology and Trans Health.

You can cancel or reschedule appointments at least 24 hours before your appointment time.
